초록
[목적]
정맥역류는 대복재정맥(GSV)의모든 부위에서발생 할수 있다.GSV의 직경은일반적으로 정맥에역류가 발생했을때 증가하며이완된 정맥의정도는 GSV내의역류된 위치에따라 다를수 있다.본 연구는GSV의 어느부분이 정맥역류와 관련하여가장 민감하고이완되었는지 조사하였다.
[방법]
본 연구는2016년 1월부터2016년 12월까지정맥부전의 징후와증상이 있는환자 99명(198개의다리)을대상으로 후향적연구를 진행하였다. 정맥역류의 존재를확인하기 위해초음파 검사를진행했고 GSV의직경을 측정했다.GSV는 4가지의부분으로 나뉘었다:복재대퇴정맥 접합부(Saphenofemoral Junction), 허벅지 중간부분(Mid-thigh), 하퇴부(Lower Thigh – LT) 및 무릎 아래(Below the knee). 환자들은 역류의 유무에 따라 두 그룹으로 나뉘었다.
[결과]
99명의환자들이 평가되었다;이중 58명은정맥에 역류가확인되었고 41명은정맥에 역류가없었다. 역류가있는 GSV의직경은 LT에서만유의하게 더넓었고(p < 0.001) 수신자 조작특성 곡선(ROC)에서 가장높은 부분은LT에서 0.642였다.LT직경의 유효수치(cut-offvalue)는 5mm (p = 0.025) 였다.
[결론]
GSV에서가장 민감하게이완된 정맥역류 부위는LT 였고 cut-off 직경은5mm이다.

Abstract
[Objective]
Venous reflux may occur in all parts of thegreat saphenous vein (GSV). The GSV diameter generally increases when venousreflux occurs, and the extent of venous dilation may vary depending on thelocation of reflux within the GSV. We examined which part of the GSV is themost sensitive and dilated in association with venous reflux.
[Methods]
We retrospectively evaluated the data of 99 patients (198 limbs) with signs andsymptoms of venous insufficiency from January 2016 to December 2016. Weperformed ultrasonography to investigate the presence of venous reflux andmeasured the diameter of the GSV. The GSV was divided into four locations: the saphenofemoral junction, mid-thigh, lowerthigh (LT), and below the knee. The patients weredivided into two groups; according to the presence or absence of reflux.
[Results]
Ninety-ninepatients were evaluated; 58 had venous reflux and41 had no reflux. The diameter of the GSV with reflux wassignificantly larger only at the LT (p < 0.001), and thehighest area under the receiver operating characteristic curve was 0.642 at theLT. The cut-off value of the LT diameter was 5 mm (p = 0.025).
[Conclusions]
TheLT was the most sensitively dilated site for venous reflux in the GSV, and thecut-off diameter was 5 mm.
